Guide to State Licensing

Printer-friendly version
  1. Print appropriate certification application information from your state website. Click Here
  2. Fill out application and mail to the FL-DOH Rad Tech Certification office with the appropriate state application fee for eligibility determination. NOTE: Do NOT include the examination fee.
  3. The FL-DOH Rad Tech Certification office will determine your eligibility for the certification for which you have applied. You will receive a letter from them informing you of your examination eligibility or the need for additional documentation within 30 days of receipt of your application.
  4. The FL-DOH Rad Tech Certification office will enter your information into the STATERHC.ORG database. Your FL-DOH Rad Tech Certification eligibility period will start at this time. You must pay ARRT for your examination at least 30 days prior to the last day of your FL-DOH Rad Tech Certification eligibility period. Each time you need to retake the examination, you must apply to the FL-DOH Rad Tech Certification office to be considered for a new exam eligibility. What is Eligibility Period? STATE ELIGIBILITY PERIOD
Your license/permit eligibility period is determined by your state licensing agency. You may attempt the exam three times within this eligibility period. You must contact your state licensing agency once this eligibility period has ended. Payment for examination must be received at the ARRT no later than 30 days before the end date of your eligibility period. This is to allow time for payment processing and examination scheduling. The exam must be completed before the end of your eligibility period.

You may schedule your exam appointment for any time during the ARRT-assigned 90-day examination window printed on the State Candidate Status Report. Examination windows begin on the Wednesday after payment is processed at the ARRT and extend for 90 calendar days. Please see Examination Handbook for details.
  5. If you are a first time user, you will need to create a personal profile on this site. If you already have a profile, skip to Step 6.

    2. Enter Zip Code, Date of Birth and last 4 digits of your Social Security Number as submitted on your state certification application.
    3. Verify the information that the state has entered. (Notify your state, not ARRT, of any necessary changes).
    4. Select a username and password, fill in other personal information and create your profile.
  6. Pay for your examination by credit card. (You may also pay with a cashier's check or money order, but you will need to mail your payment with a copy of your FL-DOH Rad Tech Certification office Notice of Eligibility Letter to the ARRT. Personal and business checks are not acceptable and will be returned.)
  7. Within approximately 7-10 business days, you’ll receive an email from ARRT informing you that your Candidate Status Report is available on your Profile page. You’ll also receive a printed Candidate Status Report and a state licensing handbook from ARRT via U.S. mail.
